Summer Fun Giveaway

Childhood memories are made of summer vacations.

We want to send one family on a little summer getaway to
Great Wolf Lodge, Kansas City.

To nominate a family or yourself the following criteria must be met:
1. Have a child with increased medical needs.
2. Reside in Nebraska or receiving medical care in Nebraska.
Other Details:
1. All nominations will be reviewed and voted on by The Love Adelyn Foundation Board. Winner will be announced on social media by June 15, 2019.
3. Funds will be provided in the form of gift cards.
4. Supporting documentation may be required to further evaluate the nomination.
Nomination process open until May 31, 2019.
