2018 Financial Assistance Program
The foundation's dream was to be able to serve families financially and this dream became reality in 2017. We are excited to offer our financial assistance program again in 2018. The generous supporters of the foundation and proceeds from The Hope Fore Little Hearts golf scramble help make this possible.
The financial assistance program utilizes a nomination process and you can nominate someone else or yourself.
To be considered for assistance, complete the form below.
The nomination process will be open from August 7 - September 7, 2018.
A few requirements to be considered:
1. The family must have a child with a medical need/condition.
2. The family/child must be utilizing a hospital located in Nebraska, Iowa, Kansas, South Dakota, or Colorado OR be a family living in Nebraska seeking treatment outside of the state.
3. The foundation has the right to ask follow up questions and may require supporting documentation.
